Based on observation and interview, the licensee did not comply with the section cited above. LPA observed required cushioning under climbing structure to absorb a fall. LPA observed a wooden walking beam in the play yard. Wooden bean was half covered with fabric material but the fabric is falling off exposing the wood and its not protected. This poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 06/27/2023

Based on observation and interview, the licensee did not comply with the section cited above. LPA attempted to test carbon monoxide detector but detector would not work.
