      Based on record review at 9:45 a.m., the center's daily attendance record for children failed to accurately reflect the children on the child care premises at any given time as 53 children were present and 49 children were signed in on the log. This was corrected during the inspection. Corrective Action: Effective 06/04/2026, S2 stated she will set an alarm for 9:00 a.m. and S8 will assist with checking daily attendance for accuracy, to ensure compliance with this regulation.

      Based on record review at 9 a.m., S1 failed to have documentation on file for Independent Contractors O2-O5, and O12-O13, that included the person's name, address, phone number, list of duties performed while at the center from 3/20/2025 to 4/16/2025. This was not corrected during the inspection. Corrective Action: Effective 4/16/2025, S1 stated she printed the forms and a note will be added to the visitor log for all Independent Contractors to check in the front office to complete a form before they will be allowed back into the classrooms, to ensure compliance with this regulation.
