The facility failed to maintain a current Employee/Contractor Roster for all child care personnel in the Clearinghouse. CCF Handbook, Section 5.1, K (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] There were two staff that had left employment with facility. One on 7/29/24 and the other on 8/9/24. Review of BGS on 10/18/24 and 10/21/24 proved that this was not completed with 5 business days as required. This was corrected 10/21/24. 5 Background Screening page 51 and 52 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. K. The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 5 days of when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 5 days of the employment termination.
