All 34 childrens records were reviewed. There were three enrolled children that had expired shot records. The Child Care Facility Handbook states on pages 59-60 that: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.       Indoor and outdoor play areas were not inspected daily for basic health and safety and/or any problems were not corrected before the play area is used by children and/or daily inspection logs were not maintained for 12 months. CCF Handbook, Section 3.1, A During the inspection, the Director was not able to find the daily indoor/outdoor physical environment logs. It was reported that they check the environment daily but just have not documented it. The facility will need to complete their daily logs for the month of December and send to the Licensing Specialist for review. The logs will need to be continued daily/monthly and kept for a period of 12 months. The Child Care Facility Handbook states on 20 that: 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. Due Date 12/19/2025

      Child care personnel, including volunteers who work 10 hours or more per month did not complete the required in-service training during the states fiscal year beginning July 1 and ending June 30. CCF Handbook, Section 4.2.7 There are currently 7 employees. All 7 employee records were reviewed. Three employees, did not complete the 10 hours of annual Inservice training during the time period of 7/1/2024-6/30/2025. The Child Care Training Handbook states on page 42 that: 4.2.7 Annual In-Service Training Upon successful completion of the 40-hour introductory training requirements, child care personnel must complete a minimum of 10 clock-hours or one CEU of in-service training annually during the states fiscal year beginning July 1 and ending June 30. All 7 employee records were reviewed. Three employees, either completed or partially completed the 10 hours of annual in service training for the licensing year of 7/1/2024-6/30/2025 but did have them documented on the required log. The Child Care Training Handbook states on page 43 that: B. Documentation of the in-service training requirement must be recorded on CFFSP Form 5268, Child Care In-Service Training Record, which is incorporated by reference in 65C-22.001(7)(i), F.A.C., and included in the child care facilitys personnel records. The applications will need to be signed by the parent/guardian and sent to the Licensing Specialist for review. The Child Care Facility Handbook states on page 61 that: 7.3 Enrollment Information The facility operator shall obtain enrollment information from the childs custodial parent or legal guardian prior to accepting a child in care. This information shall be documented on CF-FSP Form 5219, Child Care Application for Enrollment, which is incorporated by reference in 65C-22.001(7)(f), F.A.C., or an equivalent form that contains all the information required by the Department on CF-FSP Form 5219. CF-FSP Form 5219 may be obtained from the licensing authority or on the Departments website at www.myflfamilies.com/childcare. Due Date 12/19/2025

      An employment history verification was not documented and on file. CCF Handbook, Section 5.1, D There are currently 7 employees. All 7 employee records were reviewed. One employee, JM, did not have a completed 5-year work history reference check documented on a form. The Director reported that the reference check was completed but it was not documented. During the inspection, the Director documented the information from when she initially did the reference check before the employee's hire date on the employee's 5131 form. The Child Care Facility handbook states on page 51 that: D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained.

    • Background Screening Documents CCF Handbook, Section 7.4.145-06

      The Child Care Attestation of Good Moral Character was not completed at the time of initial screening or upon change in employers. CCF Handbook, Section 7.4.1, B There are currently 7 employees. All 7 employee records were reviewed. Two employees, KH and JM, signed their Attestation of Good Moral Character forms but did not date them. The Child Care Facility Handbook states on page 62 that: B. CF Form 1649A, Child Care Attestation of Good Moral Character, which is incorporated by reference in 65C-22.001(7)(a), F.A.C., must be completed for all child care personnel at the time of initial screening or upon change in employers. One child, ES, that has been at the facility for over 30 days, had out of state shot records but they were not transferred to a DH680 form.       Attendance records did not include the time of each childs arrival and departure. CCF Handbook, Section 7.5 During the inspection, one classroom did not have all the children signed in for the day. The teacher signed the children in during the inspection. The Child Care Facility states on pages 62-63 that: 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. B. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. However, child care facility personnel are responsible for ensuring that attendance records are complete and accurate. C. REMOVED. D. Attendance records must be maintained for a minimum of 12 months. E. Attendance records for Voluntary Pre-Kindergarten or School Readiness may be used, if applicable.

      The online CPR training did not include an on-site instructor-based skills assessment and is invalid for the individual. CCF Handbook, Section 4.2.4, E There were four teachers present during the inspection. Three of the teachers had completed CPR training online and did not have a instructor based skill assessment completed. The Child Care Facility Handbook states on page 42 that: E. CPR courses must include on-site, instructor- based skill assessments by a certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the facility and available for review by the licensing authority. B. By December 31, 2021, the facility shall maintain the following number of child care personnel with the CPR certification and first aid training that are on site at the facility at all times children are in care. For child care personnel to count to meet requirement in the below chart, they shall be child care personnel assigned to provide direct supervision of children in care: Total number of child care personnel in direct supervision of children to meet staff to child operating ratios: Total minimum number of child care personnel with Pediatric CPR certification on site when children are present: 0-25 staff/ there must be 3 present. Total minimum number of child care personnel with first aid training on site when children are present: 0-25 staff/there must be 3 present.
