Based on record review/interview on 5/8/2025, at 10 a.m., staff failed to supervise children at all times in the center. On 5/6/2025, at 1:30 p.m., while S1 was signed out but in the office, S3 was in the kitchen, S2 entered the restroom, closed the door behind her, and left the 9 resting children alone in two separate classrooms for approximately 10 minutes. During this time, C1, 1-year-old, got up from his cot in the 1-year-old classroom, walked through the Pre-K classroom, past the kitchen and office, and out the front entrance door of the center. Based on record review/interview on 5/8/2025, at 10 a.m., S2 and S3 failed to check by sight and circulate among 5 children, ages 3-to-4-year-old, in S2's classroom and 4 children, 1-year-old, in S4's classroom, grouped together for sleeping on 5/6/2025 from 12 p.m. to 2:02 p.m. Corrective Action: Effective 5/21/2025, S1 stated two staff will be physically present in the classroom until all lunch breaks are complete. All cots will be placed in one or both classrooms to where the staff supervising can observe the resting children at all times, to ensure compliance with this regulation.

Based on observation/record review on 5/8/2025, at 10 a.m., S1 failed to have an information form on file for O9, Independent Contractor, that included O9's name, address, phone number, list of duties performed while at the center on 5/8/2025. Corrective Action: Effective 5/21/2025, S1 stated she will print copies of the forms and keep at the visitors attendance record clipboard. Once complete, she will maintain all records in a file where it can be easily found upon request, to ensure compliance with this regulation.

Based on observation on 5/8/2025, at 9:33 a.m., a box fan cord was plugged in and loosely hanging below a wall cubby shelf, where it was accessible to 4, 1-year-old children. This was corrected prior to the Specialist leaving the classroom. Corrective Action: Effective 5/21/2025, S1 will review this regulation with staff and conduct a daily visual check of the classrooms to ensure there are no cords and strings accessible to the children. A zip tie will be added to all loose cords, to ensure compliance with this regulation.

Based on observation on 5/8/2025, at 9:30 a.m., S5 failed to ensure the manufacturer's restraint device was used while C2, 1-year-old, was seated in a high chair. This was corrected prior to Specialist leaving the classroom. Corrective Action: Effective 5/21/2025, S1 stated she will review this regulation with staff, and require that only staff place children in the equipment to ensure they are properly strapped, to ensure compliance with this regulation.

Based on observation on 5/8/2025, at 1:13 p.m., S5 failed to ensure the center's cribs were free of soft or loose bedding, including blankets, while C3, 8-months-old, and C4, 11-months-old, in cribs.
