Based on observation at 10:50 a.m., on 02/24/2025, S1 failed to meet the required child to staff ratio. The Specialist observed S2 in the kitchen preparing lunch while the children assigned to her, 11 children aged 4-years-old were alone, without a staff member in Classroom 2. This was corrected before the Specialist's departure.

Based on observations at 10:00 a.m., on 02/24/2025, S1 failed to ensure items that can be harmful to children were inaccessible to children as the Specialist observed chemical cleaning supplies on a shelf in S2's classroom, Classroom 2, within reach of the children. This was corrected before the Specialist's departure.

Based on observations at 10:00 a.m., on 02/24/2025, S1 failed to ensure all personal belongings of center staff were inaccessible to children, as the Specialist observed S2's purse sitting on her desk and within reach of the children in Classroom 2. This was corrected prior to the Specialist's departure.

Based on observations and record review at 10:00 a.m., on 02/24/2025, S1 failed to ensure the number of children using a room was based on the 35 square feet per child requirement as the Specialist observed 11 children, aged 4-years-old, in Classroom 2. Per the center's approved floor plan, the maximum number of children licensed in Classroom 2 is 9. This was not corrected prior to the Specialist's departure.

Based on observations and interviews at 10:00 a.m., S1 failed to have a labeled sheet for covering the cot or mat for each child, as the Specialist observed there were no tightly fitted sheets on the crib mattresses in the Nursery, Classroom 1. S1 stated she did not have any tightly fitted sheets for the crib mattresses. This was not corrected prior to the Specialist's departure.

Based on record review/observation at 10:00 a.m., S1 failed to ensure the center's staff and owner's daily attendance record accurately reflect persons on the child care premises at any given time. S1 was on the premises but was not signed in on the center's attendance log. This was corrected prior to the Specialist departure.

Based on observation at 10:30 a.m., S1 failed to meet the required child to staff ratio. Specialist observed S2 alone supervising 17 children: ages 2-years-old to 4-years-old, while S1 was cleaning the classroom, receiving children from parents at the door, and preparing lunch. This was not corrected before the Specialist departure.

Based on observation at 10:00 a.m., S2 failed to ensure the children were supervised at all times. The Specialist observed S2 in the 2-year-old classroom with the door closed. There was no staff in the 4-year-old classroom. The classrooms share a common doorway but S2 was not in the doorway and was unable to supervise all 17 children. This was not corrected prior to the Specialist departure.

Based on observation/record review/interview at 10:30 a.m., S1 failed to have documentation that the children were observed upon arrival for any physical conditions for possible signs of illness, infections, bruises or injuries, and document the results including an explanation from the parent or child. The last observation was documented 7/1/2024. This was not corrected prior to the Specialist departure.

705.C. Based on observation at 9:45 a.m., Licensing Division staff was not admitted into a center immediately, without delay. The Specialist arrived at the center at 9:45 a.m., rang the doorbell, knocked on the door, and called the center's phone. The Specialist was allowed in the center at 10:00 a.m.

Based on observation/record review at 10:45 a.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time. S4 was signed in as being present and on the premises, but she was not. S4 returned to the center at 10:54 a.m.

Based on observation at 10:45 a.m., S1 failed to meet the required child to staff ratio. The Specialist observed S1 supervising 10 children ages 1 year to 2-years-old. S1 could only supervise 7 children. There needed to be an additional staff in the classroom with S1. The child to staff ratio was corrected when S4 arrived at 10:54 a.m.

1919.A..B.: Based on observation/record review at 11:00 a.m., the center's meals did not contain all the components as specified under the Child Care Food Program of the United States Department of Agriculture as the children were served turkey sandwiches, chips, sweet peas, oranges, and milk for lunch. Chips do not meet the requirements for meals. S4 failed to have the weekly menu prominently posted, written or electronically, by the first day of each week and remain posted throughout the week.
