Based on observation/record review at 10:15 a.m., S1 failed to ensure the center's staff and owner's daily attendance record accurately reflected persons on the child care premises at any given time. S4 was signed in on the staff attendance log but was not present on the premises. This was corrected prior to the Specialist departure. Corrective Action: Effective 8/13/2025, S1 stated she will check time cards daily to make sure staff are clocked in and out to ensure compliance with this regulation.

She will talk with S5 about completing an walk around inspection of the van daily to make sure everything is current and the van is fine to ensure compliance with this regulation.
