    • Cribs Free of Toys and Other Soft or Loose Bedding1907.E.2

      Based on observation at 09:53 a.m., the center's cribs failed to be free of toys or other soft or loose bedding (including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges) while the child was in the crib. The Specialist observed a loose blanket covering C2, two-months-old, asleep in a crib in S5 and S6's classroom. This was corrected during the inspection. Corrective Action: Effective 04/28/2026, S3 stated she will review crib safety with all staff at the next staff meeting on 05/01/2026 , to ensure compliance with this regulation.

      Based on observation and record review at 9:53 a.m., S2 failed to have written authorization from a physician for C2 to use a positioning device. The Specialist observed C2, 2-months-old, laying on a boppy pillow in the crib. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 04/28/2026, S3 stated she will review safe sleep with all staff at the next staff meeting on 05/01/2026, to ensure compliance with this regulation.

      1911.G. Based on observation at 9:54 a.m., a pacifier was attached to the clothing of a child. The Specialist observed C1, one-years-old, with a pacifier attached to their clothing. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 04/28/2026, S3 stated she will notify all parents that pacifiers shall not be attached to clothing and review with all staff at the next staff meeting on 05/01/2026, to ensure compliance with this regulation.

The Specialist observed C3, 2-years-old, laying on a boppy pillow in the crib. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 08/13/2025, S2 stated she will ensure a written authorization is obtained by a physician prior to using a positioning device, to ensure compliance with this regulation.

      1909.G. Based on observation at 11:33 a.m., S2 failed to ensure a bib was not worn by a child while asleep. The Specialist observed C1, 5-months-old, and C2, 1-years-old, asleep in crib while wearing a bib. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 08/13/2025, S2 stated she will review safe sleep practices with all staff by 08/15/2025, to ensure compliance with this regulation.
