      1711.A. Based on observation at 10:30 a.m., S4 failed to meet child to staff ratio, as Specialist observed S4 supervising 12 children, ages 1 -2 years-old, alone. S4 could only supervise 7 children. There needed to be an additional staff in the class with S4 to supervise the additional children. S5 was observed supervising 6 infants. S5 could only supervise 5 children. There needed to be an additional staff in the class with S5 to supervise the additional child.       Based on record review at 2:00 p.m., S1 failed to have documentation for 1 of 6 staff members, S6, on the premises and accessible to children had current certification in adult, infant, and child CPR through training approved by the department. This was not corrected prior to the Specialist's departure. Corrective Action: Effective, 02/28/2025, S1 stated she will ensure all staff missing current certification in CPR will attend certification training on 03/06/2025, and has created a tracker with a calendar reminder of all expiration dates to ensure compliance with this regulation.
