1711.A. Based on observation at 10:30 a.m., S4 failed to meet child to staff ratio, as Specialist observed S4 supervising 12 children, ages 1 -2 years-old, alone. S4 could only supervise 7 children. There needed to be an additional staff in the class with S4 to supervise the additional children. S5 was observed supervising 6 infants. S5 could only supervise 5 children. There needed to be an additional staff in the class with S5 to supervise the additional child. S1 arrived at approximately 11:20 a.m. and the ratios were corrected. This was not corrected prior to the Specialist's departure. Corrective Action: Effective, 02/28/2025, S1 stated she will ensure all staff missing current certification in CPR will attend certification training on 03/06/2025, and has created a tracker with a calendar reminder of all expiration dates to ensure compliance with this regulation.
