      Based on observation, the licensee did not comply with the section cited above when the cement bike/tricycle path encircling the outdoor play space has pot holes and an uneven surface which makes riding the bike/tricycle on the uneven surface a hazard for tripping or crashing. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 07/28/2023
