      Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed a bucket container used for hand washing after classroom projects. Director was advised not to use container for handwashing due to risk of contamination. Director stated she would stop using container for handwashing and would resort to children hand wipes. POC Due Date: 03/27/2025

      Based on observation, interview, and record review, the licensee did not comply with the section cited above. Upon reviewing files LPA observed 3 out of 5 children's file did not have the form.
