Based on record review and interviews at 12:00 p.m., Laurena Conger failed to immediately notify the Parent, and Child Welfare within twenty-four hours, of the following critical incident: On 2/12/2025, at 10:51 a.m., Julie Eskind, Former Classroom Teacher, aggressively removed a child, 3-years-old, from the playhouse on the playground through the window. Julie Eskind then took the child by the wrist and walked her back to her previous location. The child was said to be crying the whole time. Based on record review and interviews at 12:00 p.m., although the center has developed and implemented a written behavior management policy that prohibits children being subjected to physical or corporal punishment, S19, (DOH 8/1/2022), used the following prohibited methods of discipline. On 2/12/2025, at 10:51a.m., S1 stated S19 aggressively pulled C1, three-years-old, by her arms through a playhouse window and then grabbed her by the wrist. S19 continued to hold C1 by the wrist, and occasionally spoke very loud to C1, from 10:51 a.m-10:55 a.m., while she was in time out.

Based on record review and interviews at 12:00 p.m., although the center has developed and implemented a written behavior management policy that addresses the centers use of time out, Julie Eskind, (DOH 8/1/2022) used a prohibited method of Time Out as length of time in time out exceeded one minute per year of age of child. A child, 3-years-old, was observed on center's video footage in time out for four minutes on 2/12/2025, from 10:51 a.m.-10:55 a.m. The child should have only been in time out for three minutes.

S10 (DOH 8/22/2024) and S13 (DOH 8/21/2024) both have worked at the center for more than 90 days. S1 stated she has a training class scheduled for 1/11/2024.
