1711.A.&B.&D.&E.: Based on observations and interviews at 9:30 a.m., S2 failed to ensure that a minimum of two staff members are present when more than four children are present at the center. During a walkthrough of the center, the Specialist observed S1 alone in the center and classroom with (5) six to ten-year-old children. Corrective Action: Effective 04/06/2026, S1 stated she will speak with S2 and remind her that when more than five children are present at the center, there must be at least two staff members on the premises to ensure compliance with this regulation.
