Based on record review at 11:30.m., S1 failed to have documentation of current CPR/Pediatric First Aid within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children for S5 (DOH: 7/2/2025). This was not corrected prior to Specialist departure. Corrective Action: Effective 10/16/2025, S2 stated she will consult with S1 upon her return to the center to develop a plan to keep track of trainings and expiration dates to ensure compliance with this regulation.

Based on observation at 10:45 a.m., S4 and S5 failed to ensure the bottle warmer in the Infant classroom was placed in an area that was inaccessible to children. The bottle warmer was placed on a mini refrigerator where children could crawl and pull up on and create a tipping hazard. This was corrected prior to Specialist departure. Corrective Action: Effective 10/16/2025, S10 stated he will find another location more suitable for the bottle warmer to ensure compliance with this regulation.

Based on record review and interview at 11:30 a.m., S11 failed to ensure that the manufacturerâs restraint was used on 9/18/2025 at 3:51 p.m. when she placed C1, 9-months-old, in her highchair. C1 fell out of the highchair hitting her forehead on the floor due to S11 not properly securing the buckles on the highchair. This could not be corrected prior to Specialistâs departure. Corrective Action: Effective 10/16/2025, S2 stated the high-chair was removed from the center due to safety issues to ensure compliance with this regulation.

This was not corrected prior to Specialist departure. Corrective Action: Effective 5/22/2025, S1 stated she will create and monitor a calendar to keep up with expiration dates to ensure compliance with this regulation.
