      Based on record review and interview, the licensee did not comply with the section cited above in that 1 out of 5 staff, Staff 1 (S1), had a mandated reporter training certificate that expired 5/11/25 while today's date is 5/28/2025, therefore the renewal of mandated reporter training has not been completed within 2 years. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 06/27/2025
