Based on observation, adult bathroom and kitchen were unlocked during inspection where cleaning solutions under the sinks and knives in drawers were accessible. On the playground, paint cans were accessible on the floor in the equipment shed. This is an immediate health, safety and personal rights risk to children in care. POC Due Date: 09/16/2022
