      Based on LPA records review and Director interview, the licensee did not comply with the section cited above. C1 does NOT have LIC627 in file. In addition, C2 does NOT have LIC701 in file and C3 and C4's LIC701 are missing parent signatures and dates which poses/posed a potential health, safety or personal rights risk to persons in care.       Based on LPA observations and staff interviews, the licensee did not comply with the section cited above. LPA observed in preschool classroom - Huggables, children's personal water bottles in a black bin laying on top of one another. In addition, LPA observed extra personal water bottles stored underneath the classroom sink. Water bottles were not stored properly which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 05/01/2026
