      Based on record review and interviews at 11:00 a.m., S1 failed to immediately notify the Parent of the following critical incident: On 10/1/2025 at approximately 12:15 p.m., S4 informed S1 that she witnessed S3 grab C1, age one-year-old, from a loft and put him down on the floor in a rough manner, possibly dropping him. S1 notified O1 at 5:30 p.m. the Specialist was unable to validate the allegation. Corrective Action: Effective 10/14/2025, S1 stated she will immediately notify parents of all allegations to ensure compliance with this regulation.
