The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed 3 children with expired immunizations records. Per Childcare facility handbook on page 59. The childcare facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form DH 680, or the Religious Exemption from Immunization form, DH 681.
