Based on record review, the licensee did not comply with the section cited above by ensuring all staff have a completed LIC 503 form in file, which poses/posed a potential health, safety or personal rights risk to persons in care. Breezy Mader did not have the bottom portion of the LIC 503 form completed by a physician. 1 newly hired staff did not have the LIC 503 form in file. POC Due Date: 11/30/2023
