Based on record review/interviews on 5/11/2026, S21 left C1, eight-year-old, unattended in the vehicle. On 5/8/2026, at 3:49 p.m., S21 arrived to the center after completing the afternoon school route. At 4:20 p.m., C1's parent arrived to pick up the eight-year-old and he was not in the center. S21 found C1 on the center's bus asleep. C1 was unsupervised for 31 minutes. Based on record review on 5/11/2026, at 12:45 p.m., the center's daily attendance record for children failed to accurately reflect the children on the child care premises at any given time. The children's attendance log for 5/8/2026 documented C1, eight-years-old, being checked into the center at 3:50 p.m., but C1 was left on the center's bus by S21, after they arrived to the center at 3:49 p.m. Corrective Action: Effective 5/11/2026, S1 stated the attendance logs will be properly maintained by staff and accurate at all time, to ensure compliance with the regulation.
