Based on record review and interview at 11 a.m., S1 failed to ensure the center’s children’s daily attendance included the time, and first and last name of the person to whom the child was released to each day. The Specialist reviewed the daily attendance records, and documentation showed on 02/12/2026, 03/18/2026, and 04/01/2026, there is no documentation of the time the children departed the center, as well as the first and last name of who the child was released to. 1811.A.&B.: Based on record review and interview at 11 a.m., S1 failed to ensure all staff maintained documentation of a CCCBC-based determination of eligibility (CCCBC) at least once during a five-year period. S1 failed to have a current eligible CCCBC determination on file for S5 as S5’s CCCBC expired on 01/26/2026. S1 stated she was unaware that S5’s CCCBC had expired, and immediately requested S5 leave the center. Based on observations at 9 a.m., S1 failed to ensure that the all cribs were free of toys and other soft or loose bedding, including comforters, blankets, sheets, bumper pads, pillows, stuffed animals and wedges when a child is asleep in the crib. The Specialist observed, C3, 6-months-old, and C4, 4-months-old, laying in a crib covered with a loose blanket. The blanket was removed from the crib, prior to the Specialist leaving the classroom. Based on observations at 9 a.m., S1 failed to ensure all infants were placed on their back inside of the crib while sleeping. During a walkthrough of the center, the Specialist observed two infants, C3, 6-months-old, and C4, 4-months-old, asleep face down in their crib. Before the Specialist departed the classroom, the children were placed on their backs. 1915.B.&C.: Based on record review and interviews at 11 a.m., S1 failed to have complete and accurate documentation of parents being notified of incidents, injuries, accidents, illnesses, and unusual behaviors. The Specialist observed documentation of previous incident reports provided which indicated that incident reports were filled out by staff at the center, but reports given failed to include the date of the incident, staff involved, time of the incident, and name and time of parent notifications indicating that they were notified of the incident that occurred at the center.
