CCF Handbook, Section 6.5, C (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] • LS observed that the medication in the one and two year old room had no parent permission slip nor a child's name labeled. LS also observed the infant room had a medication in the fridge with a prescription label ripped off and could not tell who it was prescribed to. • LS provided technical assistance from the handbook: • Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child. C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B (Section 2.1 Health and Safety, Page 3) General Requirements [SR] The two staff members failed to follow the discipline policy as both staff members are seen on video grabbing a child by both of her arms leaving a bruise. One staff member grabbed the child by both arms in the classroom while the other teacher grabbed the child when they were outside on the playground. Both teachers admitted to grabbing the child by the arm. 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies The facility is to conduct an all-staff meeting discussing the importance of following the discipline policy. A copy of the discipline policy is to be signed by each staff member and placed in their staff folder. The signature will indicated that they have read and understand the policy. A copy of the meeting notes including signature of all staff in attendance and a copy of the discipline policy is to be provided to the specialist as corrective action. CCF Handbook, Section 7.1, B (Section 19.2, number 2) Record Keeping [SR] Refer to the supplemental for the child who needs an updated record for their file. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.. Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 Refer to the supplemental for the staff member who has been in the industry since 2019, has not completed the required training in order to work in the child care industry. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child Child Care Facility Handbook Page | 40 care industry in any licensed Florida child care facility. CCF Handbook, Section 2.5.4, A The four vans that is used at the facility inspections have all expired. All vans will need to be inspected. 2.5.4 Vehicle Requirements. All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order.
