      C.: Based on record review at 2:00 p.m., S1 failed to provide documentation that S10 (DOH 11/7/2024) completed the online Mandated Reporter Training within seven days of the first day present at the center and prior to having sole responsibility for any children.

    • CPR and First Aid Certifications1723.F

      Based on record review at 1:35 p.m., S1 failed to have documentation of current certification in pediatric first aid and CPR prior to assuming sole responsibility for any children for S2 (DOH 9/24/2024). The Specialist observed S2 supervising 7 three and four-year old children alone. S6 joined S2's class at 1:40 p.m.
