Based on record review and interviews at 1:45 p.m., although S1 has a behavior management policy implemented, S18 (DOH 5/19/2025) used a prohibited method of discipline as C1, 3-years-old, was subject to physical punishment. According to video footage on 6/26/2025, at 10:25 a.m., S18 was trying to get C1 to stop running with a toy truck in the classroom. Based on record review/interviews on 5/15/2025 at 12:18 p.m., S1 failed to notify LDOE by written report within 24 hours of the following critical incident: On 5/9/2025, approximately 8:40 a.m. Terrebonne Parish Sheriffs Office came to center to investigate an incident that happened on 5/7/2025 when S22 shoved C1, age four-years-old twice inside the classroom. Corrective Action: Effective 5/23/2025, S10 stated she will complete a report whenever emergency personnel come to the center to ensure compliance with this regulation.

Based on record review on 5/15/2025 at 1:02 p.m., S1 failed to maintain documentation of a daily attendance record for Visitor's to include first and last name, arrival and departure times, and purpose of the visit. S10 stated that DCFS and Terrebonne Parish Sheriffs Office came to the center on Friday, 5/9/2025, but there was no documentation. Corrective Action: Effective 5/23/2025, S10 stated she would have all visitors stop at the front desk to sign in before entering the premises to ensure compliance with this regulation.

Based on record review/interviews on 5/15/2025 at 12:16 p.m., although the center has developed and implemented a written Behavior Management Policy, S22 (DOH 3/11/2025) failed to follow the policy when she used a prohibited method of discipline as C1, age four-years-old, was subject to physical punishment on 5/7/2025, at 4:13 p.m. S22 was observed on the centers video recording pulling C1's hand off the door and shoving C1 twice in the chest back into the classroom and yelling Stop resulting in C1 falling backwards to the floor crying. Based on observations on 5/15/2025 at 10:19 a.m., S20 failed to follow the Electronic Devices Policy, as the Specialists observed 5 infants engaged in a cartoon on the television in the classroom with S20, S12, and S13. This was corrected at 10:20 a.m. when S20 turned the television off. Corrective Action: Effective 5/23/2025, S10 stated she will remove the television in the infant room to ensure compliance with this regulation.

1715.A.1.3.Based on record review on 5/15/2025 at 11:13 a.m. S1 failed to have a hire date and first day onsite working with children date in S11's personnel file. Corrective Action: Effective 5/23/2025, S10 stated she will keep all documentation in a binder for new staff to ensure compliance with this regulation.

Based on record review on 5/15/2025 at 12:45 p.m., S1 failed to have documentation that S11 first day present at the center 3/12/2025 completed LDE Key Training Module 1 and the DCFS online Mandated Reporter Training within seven days of the first day present at the center, received additional orientation and completed the LDE Key Training Module 2 and 3 within 30 days of the date of hire.
