      Based on record review and interviews at 1:45 p.m., although S1 has a behavior management policy implemented, S18 (DOH 5/19/2025) used a prohibited method of discipline as C1, 3-years-old, was subject to physical punishment. According to video footage on 6/26/2025, at 10:25 a.m., S18 was trying to get C1 to stop running with a toy truck in the classroom. C1 did not stop, so S18 stuck her foot out in front of the toy truck C1 was running behind. This caused C1 to fall forward over the toy truck and S18's foot. Based on record review/interviews on 5/15/2025 at 12:18 p.m., S1 failed to notify LDOE by written report within 24 hours of the following critical incident: On 5/9/2025, approximately 8:40 a.m. Terrebonne Parish Sheriffs Office came to center to investigate an incident that happened on 5/7/2025 when S22 shoved C1, age four-years-old twice inside the classroom.
