      Children in the outdoor play area were not adequately supervised in that [During a transition a child was missing for approximately 90 seconds in outside play space. ]. CCF Handbook, Section 2.4.1, A General Requirements [SR] On 10/20/25, facility Director contacted Specialist that a child was unattended on playground for 90 seconds. Director and another staff member observed 2 teachers, transitioning children from playground to classroom. Once in classroom, one teacher noticed and a child was missing and informed Director who was still in hallway. Other staff were involved in search. One other staff member found the child hiding in a corner in the contained and fenced area. The time the child was unattended, and no one knew whereabouts was approximately 90 seconds. Both teachers responsible for child were terminated 10/20/25. Director conducted individual one on one training with all staff to ensure this does not happen again. 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Due Date Completed at time of inspection
