      Based on observations, record review, interviews, at 12:30 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for S4. S4 was hired and began orientation at the center on 05/12/2025, per her sign-in sheet, she worked at the center from 05/12/2025 through 06/10/2025. S1 was not able to provide an eligible CCCBC prior to the Specialist's departure. S4 vacated the premises immediately.       Based on observations and record review at 3:00 p.m., on 02/24/2025, S1 failed to ensure staff within their first 90 calendar days from their date of hire and did not have current certification in pediatric first aid and CPR were supervised until their training was completed. The Specialist observed S6 (DOH: 01/06/2025) and S7 (DOH: 12/02/2024) supervising 2 children, aged 1-year-old to 2-years-old, together. This was not corrected prior to the Specialists departure.

      Based on observations, record review, interviews, at 3:00 p.m., on 02/24/2025, S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for staff (S5, S6, and S7) prior to the staff working on the premises. S5 was hired and began orientation at the center on 01/22/2025, S6 was hired and began orientation at the center on 01/06/2025, and S7 was hired and began orientation at the center on 12/02/2024. S1 was not able to provide an eligible CCCBC prior to specialist departure. S5, S6, and S7 vacated the premises immediately.
