Based on observations and interview on 1/9/2026, although the center has a Behavior Management policy, S27 (date of hire: 7/31/2003) was observed using a prohibited method of discipline on video footage from the 3-4 year-old classroom. On 12/10/2025, at 8:00 a.m., S27 yelled at C1 (three-years-old) as he sat on the rug for circle time, and she continued to yell at him as she removed C1 from circle time and placed him in the library center. C.: Based on record review/interview on 10/25/2024 at 11:00 a.m., S1 failed to immediately notify O1 of the following critical incident: On 10/21/2024, while reviewing the center's video footage of the 3-4 year-old classroom, S1 discovered S24 used prohibited methods of discipline with C1, three-years-old. S24 grabbed C1 aggressively and yelled at her several times because she would not follow directions. S1 did not notify O1 until 10/22/2024, at 4:15 p.m. She stated she did not know that she was required to notify O1 immediately.

Based on record review/interview on 10/25/2024 at 11:00 a.m., although S23 is a mandated reporter, she did not report an allegation or suspicion of abuse/neglect of a child to the Child Protection Statewide Hotline. On 10/18/2024, S23 was in the 3-4 year old classroom as S24 used prohibited methods of discipline because C1, three-years-old, would not follow directions. S23 stated she did not realize S24 was using prohibited methods of discipline. S23 was placed on administrative leave for 3 days.

Based on observations/interview on 10/25/2024 at 11:00 a.m., although the center has developed and implemented a written behavior management policy that prohibits corporal, physical, and verbal punishment, S1 discovered on 10/18/2024 at 9:56 a.m., S24 (DOH 12/12/2019) subjected C1 (three-years-old) to the following types of prohibited methods of discipline: S24 yelling at C1 several times, picking her up, and moving her around the classroom aggressively. S24 was terminated on 10/24/2024.

Based on observations/interview on 10/25/2024 at 11:00 a.m., S24 failed to lift C1, three-years-old, using proper lifting techniques. On 10/18/2024, S24 was observed lifting C1 by one arm while standing behind C1 to move her to the cozy corner in the 3-4 year-old classroom.
