Based on record review and interview at 12:00 p.m., S1 failed to provide documentation of a CCCBC-based determination of eligibility for S3 at least once during a five-year period. The Specialist observed S3 assisting S2 in the infant classroom at the time of the inspection, and S3's determination of eligibility expired on 06/30/2025. Specialist informed S1 that S3 would have to leave the center immediately and cannot return until an eligible CCCBC is obtained.
