Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Health Requirements [SR] The facility failed to provide an incident report on the day of an incident. A child was left alone in a crib while the rest of the class went for a walk. Although the mother was told of the incident on the day it occurred, she did not receive and sign the report until the next day. The Child Care Facility Handbook states on page 55: A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs.

Supervision of children in the [infant] group was inadequate in that [a child was left sleeping in a crib while the rest of the class went for a walk in the stroller]. CCF Handbook, Section 2.4 General Requirements [SR] A child in the infant room was accidentally left sleeping in their crib while the rest of the class went for a walk in the stroller. Both teachers thought the other had checked the cribs. Another staff member entered the room and discovered the sleeping child. The two teachers will complete the DCF course Safety In Child Care Programs. The Child Care Facility Handbook states on page 12: B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location.

A safety deficiency having a moderate potential for harm to the children in care was observed. CCF Handbook, Section 3.1 Physical Environment [SR] A child in the infant room was observed to have a quarter sized piece of blue plastic mulch in her mouth when handed to the mother at pick up time by the teacher. The mother immediately removed the piece of mulch. The teacher was unaware that the child had the mulch in her mouth. The Child Care Facility Handbook states on page 20: A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months.
