Based on observation at 11:30 a.m., strings and cords were accessible to children under age 4. The Specialist observed a swing cord and two speaker cords in S5's classroom, which were accessible to 5 children, age 3 months through 10 months. This was corrected prior to the Specialist exiting the classroom. Corrective Action: Effective 3/03/2026, S1 will require opening staff to complete a walk-through of the center and look for cords and hazards to ensure compliance with this regulation.

Based on record review at 12 p.m., S5 failed to have documentation that 1 of 8 staff on the premises and accessible to children has current certification in infant, child, and adult CPR through training approved by the Department. S15, DOH: 12/5/2024, failed to have the current certification. A class has not been scheduled. Corrective Action: Effective 6/3/2025, S5 will review all CPR/FA certifciations for new staff on the date of hire to verify the training is approved by the Department to ensure compliance with this regulation.
