The vehicle alarm installed was not in working condition. CCF Handbook, Section 2.5.4 The 15 passenger vehicle that is used for transportation of children in care alarm system was not working. Appointment to has been set to have vehicle alarm system check. 2.5.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. A. All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. Documentation by the mechanic must be maintained in the vehicle. B. The maximum number of individuals transported in a vehicle may not exceed the manufacturers designated seating capacity or the number of factory installed seat belts. C. When transporting children, staff-to-child ratios must be maintained at all times. The driver may be included in the staff-to-child ratio if he or she meets screening and training requirements. D. All child care facilities must comply with the insurance requirements found in Section 316.615(4), F.S. Documentation shall be maintained at the facility. E. Child(ren) did not have a Florida Certification of Immunization (DH Form 680) or a Religious Exemption from Immunization (DH Form 681), on file within 30 days of enrollment. CCF Handbook, Section 7.1, B Specialist reviewed all ( 25 ) children files for all children enrolled. Children listed on supplemental had either missing/or expired DH680 Florida Immunization on file. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida.
