Based on record review/interviews at 10:00 a.m., S11 failed to ensure children were under supervision at all times. On 5/6/2025 at 9:55:42 a.m., C1, three-years-old, was observed on centers camera footage, walking up the ramp to buildings 2 and 3. C1 went into the teachers lounge in building 2 and was unsupervised until 9:58:33 a.m. C1 was found by S1. Corrective Action: Effective 05/21/2025 S1 stated she will retrain all staff to actively engage and supervise children at all to ensure compliance with this regulation.

Based on record review at 12:45 p.m., S1 failed to have documentation that the entire center and play yard was checked after the last child departed for the following days: 5/5/2025, 5/6/2025, 5/12/2025, and 5/13/2025. Corrective Action: Effective 05/21/25, S1 stated she will complete the daily visual check daily at the end of the day and review documentation for correct dates to ensure compliance with this regulation.

C.: Based on interviews/record review at 2:45 p.m., S1 failed to notify LDOE within 24 hours of the following critical incident : On 3/14/2025, at 3:47 p.m., O2 informed S1 that she was concerned C2, age 4-years-old, was touched inappropriately by another student on an unknown date and time. Corrective Action: Effective 4/16/2025, S1 stated she will report all reportable incidents and allegations to the appropriate agencies within 24 hours of knowledge of the incident or allegation to ensure compliance with this regulation.
