1103.A.2.C.1.&2.: Based on record review and interviews at 1:45 p.m., S1 failed to notify the Licensing Section and DCFS within 24 hours of knowledge of the following critical incident: On 3/3/2026, at 5:30 p.m., O4, C2's Grandmother, informed S1 that C2, 3-years-old, was running a fever while at the center and the teacher never notified the parent about the fever and that C2 was taken to urgent care and had a fever of 102.9. 1713.G.1.a.&2.: Based on record review and interviews at 1:00 p.m., S9 and S10 allowed C1, 3-years-old, to go and return from the restroom independently without staff as evidenced by S10's interview. C1 shut the door to the playground on her finger and sustained a finger injury requiring surgical intervention. Corrective Action: Effective 3/19/2026, S1 stated staff will be required to accompany all children under 4-years-old to the restroom to ensure compliance with this regulation.
