Based on observation at 9:33 a.m., S6 and S7 failed to ensure the manufacturer's restraint device was used while C1, infant, was seated in a feeding table. This was corrected during the inspection.

Based on observation/record review at 9:29 a.m., S4 and S5 failed to document when something is observed to include an explanation from the parent and/or child. S5 failed to document an observation of an injury. The Specialist observed a bruise on C2's, 1-year-old, right side forehead. S5 documented scratches, however, failed to include an explanation from the parent. This was corrected during the inspection.
