Based on record review/interview at 1 p.m., S6 and S7 failed to supervise C6, 8-months-old, at all times. On 03/31/2025, at 9:39 a.m., S10 and S6 observed blood in C6's mouth. S1 swiped C6's mouth and pulled out a piece of glass from a baby food jar that had fallen and broken in the classroom on 03/28/2025. Corrective Action: Effective 04/15/2025, S1 stated staff were re-trained on active supervision on 04/09/2025. Staff will continue to provide close supervision in the classrooms to ensure safe environments for the infants, to ensure compliance with the regulation.
