Corrective Action: Effective 09/03/2025, S1 stated she will set a reminder every month to send her staff a list of upcoming trainings to ensure compliance with this regulation.

      1713.A.B.C. Based on observations at 10:15 a.m., S5 (DOH 11/7/21) failed to ensure children were not under supervision at all times. S5 left six children ages two to three year olds with a parent in her classroom while she took 2 children to the restroom during the specialists walkthrough. S5 returned to the classroom at 10:17 a.m. Corrective Action: Effective 4/30/25, S2 stated she will have floaters assist teachers when they need to leave the classroom to ensure compliance with the regulation.
