1711.A.B.D.E. Based on observations at 10:00 a.m., S1 failed to meet the required child to staff ratio for infants and 1-year-old children. The Specialist observed S2 caring for 7 children, ages 4-months to 10-months-old, while S10 prepared lunch for all of the children. The required ratio for infants under one-year-old children is 5 children per 1 staff person. One additional staff was needed to meet ratio. Ratio was met at 11:25 a.m. when S10 finished serving lunch. At 10:02 a.m., the Specialist observed S11 caring for 9 children, age 1-year-old, S1 went into the classroom at 11:30 a.m.…

1719.B. Based on observations at 11:00 a.m., S1 failed to have documentation that S11 completed the DCFS online Mandated Reporter training within seven days of the first day present at the center. S11s first day present at the center was 08/13/2025. S11 completed the DCFS Online Mandated Reporter training on 09/02/2025. Corrective Action: Effective 09/30/2025, S1 stated new staff will complete the DCFS online Mandated Reporter training prior to their first day present at the center to ensure compliance with this regulation.

The Specialist observed S11 supervising 9 children, age 1-years old. This was corrected when S1 entered the classroom at 11:30 a.m. Corrective Action: Effective 09/30/25, S1 stated she will ensure all new staff complete CPR/PFA certification approved by the department prior to their first day present at the center to ensure compliance with this regulation.

This was corrected prior the Specialist departure. Corrective Action: Effective 9/30/2025, S1 stated she will check the CCCBC roster daily to ensure all staff have been added to the centers roster to ensure compliance with the regulation.

S6 was observed caring for 4 children ages two to three years old. Corrective Action: Effective 6/24/25, S1 stated she will have all new hires complete certifications within 60 days of hire and prior assuming sole.
