      Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed S1 did not have a current Mandated reporter training certificate, Director stated that staff forgot to do the mandated reporter training. POC Due Date: 04/24/2023

      Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed Facility did not have a carbon monoxide detector in any of the classroom. Director stated that director thought that facility had dual fire and carbon monoxide detector.
