This was corrected at the time of investigation in that, the new director is aware that all incidents/accidents rather staff or non staff member must be documented the day of the incident/accident. The facility has taken incident/accident writing training and will continue to document incidents/accidents. 6.4 Accident/Incident Notification A. CCF Handbook, Section 6.4, A Health Requirements [SR] The child's teacher C.M. admitted that she did not write a written incident report when the child C.L. was bitten but she did verbally tell the parent of the child he was bitten. Corrective action: The Director M.F. stated that she would have a facility meeting and discuss with staff the importance of documentation of incident/accident standards in an effort to make sure that all staff are documenting all incident/accident that occur at the facility. Child care personnel inappropriately interacted with children in care in a manner that was aggressive, demeaning or intimidating in nature. CCF Handbook, Section 8.2, C Enforcement [SR] Teacher H.P. was observed on video inappropriately with child L.C. when she aggressively grabbed child L.C. underneath his arms and forcibly sitting him in his chair. 8.2 Child Safety A. Acts or omissions that meet the definition of child abuse or neglect provided in Chapter 39, F.S. or Chapter 827, F.S., constitute a violation of the standards is section 402.301-.319, F.S., and will support imposition of a sanction, as provided in Section 402.310, F.S. B. Failure to perform the duties of a mandatory reporter pursuant to Section 39.201, F.S., constitutes a violation of the standards in Section 402.301-.319, F.S. C. Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. A method of discipline was used at the facility that was severe, humiliating or frightening to children in that [Staff member K.G. forcibly slammed child A.T. onto her cot causing an injury to the child's mouth.]. CCF Handbook, Section 2.8, A and s.402.305 (12), F.S. General Requirements [SR] The facility's video showed staff member K.F. forcibly holding child A.T. on her sleeping mat, then when child A.T. was attempting to get up staff member K.F. grabbed child and slung her on the mat causing an injury to the child's lip. This was corrected at the time of incident in that, the video was being observed by the father of the child, Police Officer, Director, and Owner and once viewed the Police Officer arrested the staff member immediately. 2.8 Child DisciplineA. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies. C. CCF Handbook, Section 6.4, B & D Health Requirements [SR] The facility is being cited as they were not able to produce a copy of the incident/accident report because they stated it was given to the Police Officer, however the father of the child A.T. stated he did not receive an incident/accident report but a disenrollment report from the Director. CCF Handbook, Section 8.2, C Staff member J.W. was observed inappropriately interacting with child C.A., in a matter in a matter that was aggressive, demeaning or intimidating when she picked up childs C.A. cot and drugged it across the room with the child still laying on it then slammed the cot down and sat in a chair next to cot and put her leg across the childs C.A. body so child could not move. This was corrected at the time of incident J.W. admitted to she could have done better and has taken two classes, to ensure it does not occur again. 8.2 Child Safety A. Child care personnel including volunteers who work 10 hours or more per month did not complete the 40 hour Introductory Child Care Training requirement. CCF Handbook Section 4.2.1 Counselor observed a staff member (J.D.), had an industry starting date of: 06/28/21. Had not completed the required 40 hour introductory child care training. This was resolved when the facility director told the staff to return after completing the required training. 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Counselor reviewed a personnel file, (G.G.), had completed the 90 day preservice training after the required 90 days of employment This was resolved as the employee has since completed the required training. Employee has an industry start date of: 07/05/2023 Completed the 90 day introductory training on 12/13/2023. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child are industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The childcare program is responsible for obtaining training documentation from child care personnel.
