Based on LPA record review, the licensee did not comply with the section cited above in 3 out of 3 records reviewed in which the facility is not maintaining 15 min sleep log checks. Staff are present in the room when children sleep but are not recording the checks which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 05/14/2026
