Based on interviews and record review at 1:45 p.m., S1 failed to notify the parent immediately and the Licensing Section and DCFS within 24 hours of the following critical incident : On 12/3/2025, at 7:47 a.m., C2âs parent witnessed S28, Lead Classroom Teacher, yelling at and pulling C1, 4-years-old, up by his jacket in the classroom, during drop off time. The parent reported the incident to S2 on 12/3/2025, at 7:50 a.m. The Department and DCFS were not notified about the allegation. The parents were notified on the morning of 12/4/2025.
