An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] The outdoor fence had a missing board and was observed to not be in good repair. Technical assistance given: CCF Handbook, Section 3.1, A7.6 Outdoor Time, Fencing and Play Area Requirements A. Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards. B. Tubs, buckets, and other open containers of water should be emptied immediately after use. C. The outdoor play areas must be enclosed with fencing or walls a minimum of 4 feet in height. The fence must not prevent the supervision of children. Due Date 3/27/2026

The facility's transportation log did not include [Departure location, Destination, Time of Departure, and Time of Arrival]. CCF Handbook, Section 2.5.2 General Requirements [SR] A new/blank Transportation Log was obtained and completed with all required elements, which also made it easy to distinguish which children were marked on/off the vehicle. Technical support was provided. 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable).

The facility's use of seat belts was not age appropriate for children being transported who required safety restraints. CCF Handbook, Section 2.5.5 General Requirements [SR] Licensing Specialist observed JA pull up to the CCF in the CCF van. She got three younger looking children off the van and took them inside the CCF. LS observed TA take two booster seats from the floorboard and place them on the front seat while conducting a visual sweep of the van. She said the children were sitting in them while being transported to the CCF. However, she couldn't explain why they were on the floorboard. Three children (two 1 y/o's and a 3 y/o) were transported in the child care facility (CCF) van without being placed in the required/appropriate safety restraint/car seats. Four appropriate car seats were purchased the same day to transport children (1 - 3 y/o's); in addition to three booster seats for 4 y/o's. Technical support was provided (also regarding the proper way to secure the car seats on the van seat and expiration dates of the car seats). 2.5.5 Seat Belt/Child Restraints Each child, when transported, must be seated in a back seat in an individual factory installed seat belt or federally approved child safety restraint. The child safety restraint must be installed, secured and used in accordance with the manufacturers instructions and a copy of such instructions must be maintained (in the vehicle and/or on file). CCF Handbook, Section 4.2.4, B (Section 13.4, number 2) Health Requirements [SR] When LS arrived 15 children were present with two required staff. Only one of those staff had current CPR. The Director, who had current CPR/First Aid, arrived a few minutes later. The CCF was then compliant with CPR/First Aid requirements. Technical support was provided. Director stated that the 2nd staff is taking CPR/First Aid training in a week. She said she will come in earlier to comply with CPR/First Aid requirements until the other staff completes CPR/First Aid training. B. By December 31, 2021, the facility shall maintain the following number of child care personnel with the CPR certification and first aid training that are on site at the facility at all times children are in care. Documentation of pediatric cardiopulmonary resuscitation (CPR) certification and first aid training must be kept on file and available for licensing to review. Child care personnel satisfying the certification and training requirements shall be present at all times that children are in care at the facility, and at least one child care personnel with the CPR certification and first aid training on field trips which includes all activities away from the program, and during all transportation activities. D. Certificates of course completion are valid based on the time frames established by each first aid training and CPR certification program, not to exceed three years. E. CPR courses must include on-site, instructor- based skill assessments by a certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the facility and available for review by the licensing authority.

An employment history verification was not documented and on file. CCF Handbook, Section 5.1, D The two staff listed on the Supplemental Sheet did not have Employment History Verifications. Technical support was provided. D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained.

The credentialed director was not on-site a majority of the hours that the facility is in operation as documented on timesheets, personnel schedules or employment records. The facility's driver did not have [ Valid certificate(s) of course completion for first aid training and pediatric cardiopulmonary resuscitation (CPR) procedures]. CCF Handbook, Section 2.5.1 General Requirements [SR] The driver's CPR did not include a skills assessment. Another staff member with valid CPR/First Aid training will ride the vehicle while transporting children until the driver can obtain CPR with a skills assessment. Technical support provided. 2.5.1 Driver Requirements The driver of any vehicle used by a childcare program to provide transportation must have the following: B. An annual physical examination which grants medical approval to drive, and valid certificate(s) of course completion for first aid training and pediatric cardiopulmonary resuscitation (CPR) procedures. E. CPR courses must include on-site, instructor- based skill assessments by a certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the facility and available for review by the licensing authority.

The vehicle alarm installed was not in working condition. CCF Handbook, Section 2.5.4 Director submitted a safety plan in the meantime while the vehicle alarm is checked out/repaired. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2.
