Based on record review and observation at 9:00 a.m., S1 failed to ensure the center's staff and owner's daily attendance record accurately reflected persons on the child care premises at any given time. S4 and S6 were on the premises, but were not signed in on the attendance log. This was corrected prior to the Specialist's departure. Corrective Action: Effective 5/18/2026, S1 stated she will check the staff attendance log twice daily to ensure it is accurate to ensure compliance with this regulation.

1713.A. Based on observation at 9:10 a.m., S6 failed to ensure the children were supervised at all times. Upon entrance into building #2, S6 was asleep. There were 8 children in the building, ages 2-3-years-old. 7 of the children were sitting at the table and 1 child was sitting behind a shelf. This was corrected prior to the Specialist's departure. Corrective Action: Effective 5/18/2026, S1 stated she will talk to S6 about the supervision policy. She will also complete a walkthrough of the center every 2 hours to ensure supervision is met to ensure compliance with this regulation.

Based on observation/record review at 11:30 a.m., S1 failed to ensure the daily attendance record for staff and owners is maintained and accurately reflects the staff members and owners on the center premises at any given time. S1 and S4 were not on the premises but were signed in. S6 was on the premises but was not signed in. This was partially corrected prior to the Specialist departure. Based on observation at 11:15 a.m., S5 failed to meet the required child to staff ratio. The Specialist observed S5 supervising 7 children: ages 1-month-old to 2-years-old. Corrective Action: Effective 4/17/2025, S1 stated she will make sure there are two staff in the classrooms at all times to ensure compliance with this regulation.

Based on record review/observation at 11:45 a.m., S1 failed to have documentation for 1 of 10 staff members on the premises and accessible to children had current certification in adult, infant, and child CPR through training approved by the department. S5 failed to have the current certification. This was not corrected prior to the Specialist departure. Corrective Action: Effective 4/17/2025, S1 stated S5 has training scheduled for May 13, 2025. She make sure that she is not left in a class alone to ensure compliance with this regulation.

Based on record review/observation at 11:45 a.m., S1 failed to have documentation for 1 of 10 staff members on the premises and accessible to children had current Pediatric First Aid through training approved by the department. S5 failed to have the current certification. This was not corrected prior to the Specialist departure. Corrective Action: Effective 4/17/2025, S1 stated S5 has training scheduled for May 13, 2025. She make sure that she is not left in a class alone to ensure compliance with this regulation.

C.: Based on record review at 11:15 a.m., S1 failed to have at least one staff member on the premises trained in medication administration. This was corrected when S1 and S4 arrived at the center 11:34 a.m. Corrective Action: Effective 4/17/2025, S1 stated she will have the remaining staff take the medication training online to ensure compliance with this regulation.

Based on record review at 11:30 a.m., S5 failed to have documentation that included the liquid intake, food intake, disposition, bowel movements and eating and sleeping patterns for C1 (3-month-old) and C2 (one-month-old). Corrective Action: Effective 4/17/2025, S1 stated she will check the daily infant reports twice daily to make sure they are being done to ensure compliance with this regulation.

Based on observation/record review at 9:15 a.m., S4 failed to ensure the daily attendance record for staff and owners is maintained and accurately reflects the staff members and owners on the center premises at any given time. Upon arrival at the center S3 was present and on the premises. S3 failed to sign in and out on the center's attendance log. This was corrected prior to the Specialist departure.

C.: Based on record review at 10:00 a.m., S4 failed to have documentation that S9 and S10 completed center-specific orientation to the policies and practices of the center and DCFS online mandated reporter training within 7 days of the first day present at the center. S9's hire date and first day present at the center was 2/10/2025. Center-specific orientation should have been completed by 2/17/2025. S10's hire date and first day at the center was 3/17/2025. C.: Based on record review at 10:00 a.m., S4 failed to provide opportunities for continuing education of staff members who are left alone with children, or who have supervisory or disciplinary authority over children. 1 of 4 staff did not have 12 continuing education hours. This was not corrected during inspection.

C.: Based on record review at 10:00 a.m.,S4 failed to have at least one staff member present trained in medication administration whether the early learning center administers medication or not. The center has four staff members trained in medication administration training, but 0 of 4 staff were present at the center. This was corrected when S4 arrived at the center at 9:30 a.m.

Based on record review at 9:30 a.m., S1 and S9 failed to have accurate documentation that included the liquid intake, food intake, disposition, bowel movements and eating and sleeping patterns for one infant, 3 months old. The daily infant reports were already completed for the entire day of 3/25/2025.
