      2.: Based on observations at 9:45 a.m., S1 failed to ensure a manufacturer's restraint device was available to be used by children in a one-year-old classroom and in the feeding area of the center. The Specialist observed two feeding tables with no restraint devices attached to the seating. The tables are used for feeding in the classroom and in the lunch area. The Specialist observed one 4-month-old infant in the crib asleep with a bib around his neck. Corrective Action: Effective 12-16-2025, S1 stated she will have a meeting to retrain staff on policies and procedures of the infant classroom, to ensure compliance with this regulation.

The Specialist observed documentation of two completed Tornado drills for the Month of April 2025. Corrective Action: Effective 6/11/2025, S1 stated she will set Monthly reminders, to ensure compliance with this regulation.
