      Based on observations, on 12/10/2024, at 11:00 a.m.., S7 failed to maintain close proximity or accompany children while they were walking to the restroom. The Specialists observed C11 (three-year-old child), out of S7's sight. S7 did not provide adequate supervision for C11 while the child walked alone down the hallway to the restroom.

Based on observations/record review/interviews, at 11:30am, S2 failed to notify the Department and Child Welfare within 24 hours, and the parent immediately of the following critical incident: On 10/07/2024, at 11:30pm, S10 yelled at C1 while grabbing him by the ankles, pulling him from underneath the slide. S10 picked C1 up underneath his arms, moving into an undisclosed area, for time-out. In a separate incident on the same day at 8:52 am, S10 yelled at C2 and C3 to line up, picking C3 up by both arms placing her in line.

While reviewing video footage for 9/26/2024 , the Specialists observed S10 pull C1, four-year-old, by his ankles, pulling him from under a slide, picking him up underneath his arms, moving into an undisclosed area, for time-out. In a separate incident on the same day at 8:52 a.m. , 10 yelled at C2 and C3 to line up, picking C3 up by both arms placing her in line.

      Based on observation at 11:30am., S10 failed to use proper techniques for lifting a child, by not lifting C3(two-years-old) under her arms. While reviewing video footage of 9/26/2024, at 8:52am., the Specialists observed S10 lift C3, two-year-old, by her upper arms from the backside.
