      Supervision of children in the [VPK class] group was inadequate in that [the former teacher forced a child into the bathroom and appeared to lock the door and then she leaned on the door to prevent child from coming out]. CCF Handbook, Section 2.4 General Requirements [SR] During the investigation licensing reviewed the video and the saw one of the former VPK teachers take a child and put them in the bathroom with the light off and pretend like she locked the door, (the door doesn't lock but has the turn knob so it looked like door was locked) and then teacher rested her body up against the door preventing child from coming out. Technical Assistance given: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Corrective Action: Provider immediately fired the two teachers that were in the room. Due Date Completed at time of inspection

      Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B General Requirements [SR] Teacher in the VPK classroom did not follow their own discipline policy. On October 29, 2025, on this child's first day the VPK teacher listed on the confidential page put the child in the bathroom instead of sitting the child in time out which is what their discipline policy states. Technical Assistance: 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies. Corrective Action: All child care personnel must take Basic Guidance and Discipline and send via email a copy of all personnel's certificate of transcript showing this class was taken.       The play equipment was not maintained in a safe or sanitary condition for the children to use in that [the blue metal sit on truck is rusty and some of the paint was peeling off.]. CCF Handbook, Section 3.12, B and F (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] During the inspection Licensing Specialist observed a blue metal sit on truck is rusting and the blue paint is peeling off. The blue metal truck is on the one- and two-year-old playground. Technical Assistance given from handbook: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. C. All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Corrective Action: Provider must submit pictures via email showing that the blue metal truck has either been painted or been removed from the playground.

      Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Record Keeping [SR] Child Abuse and Neglect forms had not been updated annually by 7 staff members, 6 were completed at time of inspection. One staff member listed on the confidential page was not at center yesterday to update form. Provider will have employee sign a new Child Abuse and Neglect form and send that form via email to FSC. Technical Assistance given from Handbook: 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.
