Kitchen manager labeled the opened packages.The Child Care Facility Handbook states on page 30:D. CCF Handbook, Section 3.9.2, D (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] Specialist observed in both the storage area and the freezer walk-in that there were open containers of food not properly dated. The kitchen staff labeled and dated food containers. The Child Care Facility Handbook states on page 31: D. Opened packages of perishable or leftover food items must be properly covered or sealed in containers or bags, labeled with the date, and properly stored and discarded within seven calendar days. Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] Specialist observed infant bottles that were not labeled with first and last names. The director and teachers labeled all bottles while specialist was completing this inspection. The Child Care Facility Handbook states on page 34: B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed all 98 children files. Specialist found one shot record expired. Administrative assistant called parent to have the child picked up or bring in new shot records. The Child Care Facility Handbook states on page 59: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. The Student Health Examination or equivalent health statement was not acceptable in that: [ It was not current (over two years old)]. CCF Handbook, Section 7.2 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed all 98 children files. Specialist found one child's student health examination form expired. Administrative assistant called parents to come pick up the child or bring in updated student health form. The Child Care Facility Handbook states on page 60: 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A minimum distance of 18'' was not maintained between each napping/sleeping space. CCF Handbook, Section 3.6.2, C Physical Environment [SR] Specialist observed children sleeping on mats that were less than 18 inches apart. Teachers moved the children and their mats. The Child Care Facility Handbook states on page 25: 3.6.2 Nap/Sleep Space C. A minimum of 18 inches must be maintained around individual napping and sleeping spaces. A maximum of two sides of a napping or sleeping space may be against a solid barrier, such as the wall. The solid side of a crib does not meet the requirements for a solid barrier.

Child care personnel working in the food preparation area did not wear proper head covering. CCF Handbook, Section 3.9.1, B Food and Nutrition [SR] Specialist observed the Kitchen Manager in the kitchen without a hair net and gloves. Kitchen Manager promptly put on gloves and hairnet. The Child Care Facility Handbook states on page 29: B. Child care personnel, while working in the food preparation area, must wear proper head covering, such as a hair net or hat. To prevent contact with ready-to-eat foods, child care personnel must use clean disposable gloves, utensils, or similar items in the food preparation area.

Food was stored in an inappropriate area. CCF Handbook, Section 3.9.2 Food and Nutrition [SR] Specialist observed in the storage area. Open packages of dried goods open that were not labeled properly. Kitchen Manager proceeded to label and date packages. The Child Care Facility Handbook states on page 30: D. Opened packages of perishable or leftover food items must be properly covered or sealed in containers or bags, labeled with the date, and properly stored and discarded within seven calendar days.

Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Record Keeping [SR] Specialist reviewed all 18 staff files. One staff member had an expired Background Screening. Staff member will be allowed to work until the end of the day but must not return until they have a cleared and updated screening completed. The Child Care Facility Handbook states on page 61: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review.

The facility owner failed to notify the licensing authority within five working days of when the facility lost the credentialed director or when there was a change of director. CCF Handbook, Section 4.7, D. 1 and 2 Facility did not properly inform DCF Childcare licensing within 5 days of the previous director leaving, and new director took over. Childcare Facility Handbook states on page 46: F. Childcare facility owners must notify the licensing authority within five working days of when the facility loses a credentialed director or when there is a change of director. 1. The licensing authority will then issue a provisional license for a period not to exceed six months for any facility without a credentialed director. 2. The provisional license will have an effective date of the first day the facility was without a credentialed director.

An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Physical Environment [SR] Counselor found rust and chipping paint on the playground used for VPK children. There was also a crack in the slide. the Child Care Handbook states: 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months.

The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C Counselor discovered staff member JR did not complete their early literacy training within the allotted time frame. 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry

The director responsible for the daily operation of the program did not have an active Director Credential. CCF Handbook, Section 4.7 Counselor found the facility to not have a director with an active director credential. The Child Care Facility Handbook states: 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license.
