      1507.E. Based on record review on 11/19/2025, at 11:30 a.m., the center's daily attendance records for visitors failed to reflect when a DCFS representative was on the premises accurately. S1 stated that on 11/7/2025, a DCFS Representative visited the center to investigate allegations of suspected abuse/neglect and failed to sign the visitors' log. S1 stated the DCFS Representative was asked to sign in, but did not. Corrective Action: 11/19/2025 S1 stated she will ensure S2 includes the visitor log for review at the end of the day to ensure compliance with this regulation.

      1509.A.8.a&b.: Based on record review and interview at 11:45 a.m., although the center has implemented a behavior management policy, S25 (DOH 11/19/2024), used a prohibited method of discipline. On 11/03/2025 at 1:02 p.m., while C2 (2-year-old) was lying on her mat, S25 roughly rolled C2 by her left arm and leg, placing her on her stomach. Then, she forcibly pulled C2’s mat next to her and harshly said, “Be Quiet, Hush.” S25 also grabbed C1's (2-year-old) blanket and rolled him back onto his mat.       Based on record review on 6/24/2025, at 1:00 p.m., S1 failed to provide documentation that 1 of 23 staff had current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility. S15 was observed in the classroom alone with 10 children, aged 2-3 years-old. Corrective Action: Effective 6/24/2025, S2 stated a CPR and Pediatric first aid class will be scheduled immediately to ensure compliance with this regulation.

      Based on record review, S2 failed to provide documentation, which shows that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The end-of-day check had not been completed since 6/13/2025. Corrective Action: Effective 6/24/2025, S2 stated she will complete the visual check each day to document that no child has been left unattended at the center to ensure compliance with this regulation.

      Based on observations/interview on 4/7/2025, although the center has a behavior management policy that prohibits corporal and physical punishment, S3, (DOH: 6/06/2019), used a prohibited method of discipline on 3/31/2025, at 4:11 p.m. The Specialist reviewed video footage from the two-year-old classroom, and observed S3 yell at C3 (two-years-old) to return a toy.       Based on observations/interview on 4/7/2025, children failed to be under supervision at all times by S3 (DOH: 6/06/2019) as on 3/31/2025, at approximately 4:11 p.m., S3 gave C2 a snack and directed her to go inside a closet within the classroom, where she remained for several minutes alone. S3 pushed the closet door, only leaving a small opening while C2 remained inside.
