      C.: Based on record review/interview at 11 a.m., on 04/02/2025, S1 failed to notify the Department within 24 hours of the following alleged critical incident: On 03/24/2025, C1, 8 months old, arrived to the center at 8:25 a.m., and O1 notified the center that C1 was fussy and was attempting to make a doctors appointment. C1 was picked up by O1 from the center at 9:55 a.m. to go to the doctor, and S4 notified O1 that C1's leg looked swollen. Based on interviews/record review on 12/10/2024, S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 11/14/2024, O2 alleged to S1 that C1 had a bruise on his ear and that C1 accused S4 of pulling his ear while outside on the playground.

According to video footage, on 11/14/2024, S4s class was playing on the ramp. S4 walks over to an unidentified child (S2 was unable to identify him) and grabs him by the left ear, leading him to the other side of the ramp.
